New Student Registration

To register your student at West Perry School District, you must complete the online enrollment form and make an appointment at the District Office. The District Office is located at 2606 Shermans Valley Road, Elliottsburg, PA 17024. All steps must be completed for your student to be registered at West Perry School District.

  1. Online Pre-registration - A parent or guardian must pre-register their child(ren) by completing the online enrollment form.
    • The online enrollment form is completed using InfoSnap. If this is your first time using InfoSnap, you will need to create an account.
    • An online enrollment form must be completed for each child. After the first child is added, you can click the link to 'Add' an additional child.
    • For the 2017-18 school year, access the enrollment form here.
    • If you do not have internet access or a computer, you can complete the online enrollment form during the on-site registration appointment. (Please note that you will need to allow more time for the on-site registration appointment.)
  2. On-site Registration - After completing the online enrollment form, contact the District Office to schedule an on-site registration appointment. You may contact the District Office by phone at 717-789-3934 x5500 or via email to rlauzon@westperry.org.
    • All children in the family can be registered during the same on-site registration appointment.
    • You must bring the following documents to your scheduled appointment: child's birth certificate, immunization records, proof of residency and parent/guardian's photo ID. Details regarding each item are provided below.

**Children registering for Kindergarten for the 2016-2017 school year must turn five (5) years old before September 1, 2016.
**Children registering for 1st grade for the 2016-2017 school year must turn six (6) years old before September 1, 2016.

What to bring when you register your child

  • Proof of your child's age: Any one of the following constitutes acceptable documentation: original birth certificate, notarized copy of birth certificate, a valid passport, notarized/certified record showing the date of birth, notarized statement from a parent or relative indicating the date of birth, prior school record indicating the date of birth.
  • Immunizations required by law: Acceptable documentation includes: the child's immunization record, a written statement from a former school district or medical office, verbal assurances from the former school that the required immunizations have been completed with records to follow.
  • Proof of residency (two required): Acceptable documentation includes: a deed, lease, sales agreement, notarized statement from landlord, mortgage information, driver's license or  DOT identification card, utility bill, current credit card bill, permit to carry card, vehicle registration. If residing with a district property owner, the property owner must sign a notarized "multiple occupancy form."
  • Photo Identification: This is required in order to notarize documents at the registration appointment such as sworn statement attesting if the student has been or is suspended or expelled for offenses involving drugs, alcohol, weapons, infliction of injury or violence on school property.
  • Other documents: These documents are necessary in order to appropriately schedule students for classes. Report cards/transcripts (required for students in 9-12), all Special Education documents (IEP, ER, RR, NOREP), Service Agreements (504), Gifted documents, attendance records, any other records relevant to your child's education.

Who can register a child

Parents or court appointed guardians living in the West Perry School District may register a student. The parent/guardian must come into the District Office in person to complete the registration process. (Note: Exceptions apply to homeless youth according to the McKinney-Vento Act, migrant and foster students.) A student living with someone other than a parent will be registered in accordance with guidelines set by PA Public School Code section 24 P.S. 13-1302.

Residents, other than the parent or court appointment guardian, will be asked to sign an affidavit stating that they and the student live in this District on a full-time basis.


Affidavit of Multiple Occupancy

If you are registering your children at West Perry School District and you are residing with a district property owner, this form should be completed by the owner or lessee of the property.

Application For Multiple Occupancy Registration

This form should be completed by the parent/guardian of the children enrolling in West Perry School District, if the family is living in a home within the West Perry School District that is owned or leased by another person.
